This is a contract between #####, the owner of #####, and

_____________________, the lessee.

1) Amount of Lease: Lease will be in the amount of $525/wk and continuing until ended by the agreement of both the lessee and the owner. The lease will be paid in advance for the following week. The lease amount will be due every week afterward. There is no grace period. Any increase because of TLC rate increases or a new car hack up will be agreed upon by both parties and will start one (1) week later.

2) Late Fee: There is no late fee. However, the owner reserves the right to turn off the meter in the case of non-payment by the due date.

3) Release from Contract: Owner agrees to not break this contract for a period of _____________, commencing on ____________. The lessee agrees to give two week’s notice prior to breaking this contract.

4) Payment location: Driver will deposit the lease amount into any Commerce Account bank. The account number is ########, and the name on the account is ‘_______. The receipt that the teller gives will be the proof of payment, and any disputed payments will require the deposit receipt as proof of payment.

5) Credit Card Transactions: Driver will multiply the amount of the credit card transactions by .95, and then deduct that amount from the weekly lease. Should the credit card transactions be more than $525 at the end of the week, the owner will either issue a check or credit the amount to the next week, at the driver’00s option.

6) TLC Inspections: The lessee will attend all TLC inspections, with no credit given for time spent there. Within two days of the inspection, the driver will bring the car into the garage for servicing so that the car will pass inspection.

7) Repairs/Maintenance: Maintenance and upkeep like oil changes, brakes, tires will be preformed without credit for time for the lessee but at the owner’s expense at __________. Regular oil changes every 4000miles or once every two weeks, whichever comes first, is required. Repair time up to 12 hours per week (1 Shift) will not be credited. Repairs requiring more than 12 hours per week will be credited at $75/shift.

8) Accidents: For towing covered by the owner, the garage’s tow truck will be used, the number is 1-800-506-4002; any other tow company expenses will be covered by the lessee. The lessee will be responsible for any amount of damage not attributed to normal operating use of the car up to the insurance deductable.

9) Trip Sheets, Paper Rolls, Windshield Fluid, and Car Washes: Lessee will wash car at his own expense, and be responsible for windshield fluid and meter rolls. Any “dirty car” tickets issued by the TLC will be paid by lessee.

10) Change in Residence: Any change in the residence of the lessee will be reported to the owner, in case provisions of section (3) occur.

11) EZ Pass Required: Lessee will provide his own EZ pass. Any non-payment of toll tickets will be sole responsibility of lessee.

12) Defensive Driving Course Required: Lessee will agree to take the defensive driving course offered by Pearland Insurance, at owner’s expense, within 1 month of leasing car, if the he/she does not currently have a valid defensive driving certificate.

13) The use of 3T22A as a cab will be only by the listed driver and no other hack licensee. Any other hack licensee will not be listed on the insurance, and will not be covered in case of accidents or TLC tickets. Any tickets or accidents charged to anyone not listed in this contract will be the sole responsibility of the lessee.

14) Car will be shifted 5am-5pm, and returned to______________. Driver must notice the parking signs, and park on the side that will not earn a ticket on Thursday or Friday.
